Please be informed that there are some changes for the IC application.I will up date the latest information later. This information is ONLY meant for Malaysians who are going to apply for new identity card (IC), renew thier old IC or apply for replacement of IC at the registrar office. DOCUMENTS REQUIRED BY THE REGISTRAR OFFICE:  For new applicant who reaches the age of 121) Original and a copy of the applicant's birth certificate 2) Original and a copy of the IC of both parents 3) No IC photograph needed, as the departments are using digital camera to take the IC photo 4) Fee: RM4.00 in cash There will be a late penalty if the applicant does not apply for new IC on time. Application made between 12 years & 30 days old to 13 years old --RM 1.00 13 years old to 14 years old -------------RM 2.00 14 years old to 15 years old -------------RM 4.00 15 years old to 16 years old -------------RM 6.00 For renewal of IC at the age of 181) Original and a copy of the applicant's birth certificate 2) Original and a copy of the IC of both parents 3) No IC photograph needed, as the departments are using digital camera to take the IC photo 4) Old IC made at the age of 12 5) Fee: RM4.00 in cash Replacement of IC due to loss1) Police Report(which requires 8 or 9 pieces of IC photos) 2) No IC photograph needed, as the departments are using digital camera to take the IC photo 3) Original and a copy of the applicant's birth certificate A penalty will be imposed for loss of IC: Loss for the 1st time -----RM 10.00 Loss for the 2nd time ----RM 30.00 Loss for the 3rd time ----RM 50.00 Loss for the 4th time -----RM100.00 Replacement of IC due to damage1) No IC photograph needed, as the departments are using digital camera to take the IC photo 2) Original damaged IC 3) Penalty: RM 10.00 |
