Found in the Great Glacier. From the mountain crossroads near the upper right of the glacier (this is the area where about four paths cross at the foot of a mountain range), go up to the upper right path. It will be on the second screen.
Additional Notes
You can use Added Cut only with spells, not commands. Sorry, but Double Cut + Added Cut is not a materia combo that works.
Actually, Added Cut can be used with a Master Command, but can only hit one target even with a Mega All. With a Master Command, Added Cut works with Steal, Sense, Manip., Item, Slash-All, D.blow, Morph, Coin, Attack, 2x-Cut. It will not work with Throw, Mime, Summon, Magic, or E.Skill.

Additional Notes
If Added Effect linked with a spell materia is placed on a character's weapon, an attack might cause a special effect to be used, such as Stop or Confu. If this is placed on the armor, the character is protected 100% from the same effect being used on him. You might also want to use Slash-All or Mega-All to have an attack hit everybody, each with a chance of having the Effect.
You can check what the effects of an Added Effect is by accessing the character's Status menu, then pressing the [OK] button two times. It will give you a list of all the effects an attack will do and all the effects the character is protected from.

The success rate of the Added Effect is most probably determined by the level of the spell materia used, as Added Effect has only two levels, but this is only speculation. Another suggestion is that a mastered Luck Plus lets Added Effect work almost 100% of the time. Also keep in mind that if an enemy is already immune to an effect, Added Effect won't change that.

Found in the Beginner's Area at Midgar, Area 5 Slums. You can also get it by trading in Coupon C from floor 63 of the Shinra Building. It is also found at Mt. Nibel.

Additional Notes
This materia won't work with all spell materia. They have no effect on Shield, Ultima, Exit, Comet, FullCure, and the Resist spell of the Heal materia.

Where to find this
This is hidden within the Northern Cave (aka Crater). After going through the brownish/organic-looking room with the Magic Pots and Movers, you will come to a large green room with a bright light emanating from the bottom. The Counter materia is hidden behind this bright light, barely visible. Just go down there and pick it up.
You can also win this materia from the Chocobo Races at the Gold Saucer.

Additional Notes
The higher the level of this materia, the more often the Counter is used. At the master level, Counter always happens.
Counter works with Attack, Manip., Throw (random weapon), Deathblow, Mime, and Morph.

Elemental

Where to find this
You'll get an Elemental materia by guessing Mayor Domino's password correctly on the first try.
Can be found in Mt. Nibel Cave.

Additional Notes
This is how Elemental works. You need to link it with a spell materia that is based on a certain element (i.e. Fire, Ice, Lightning, Gravity, Holy power, etc.).
When this link is on a weapon, the attack will also be element-based. If you had Elemental linked with Ice, and you attacked an enemy, then the attack will do extra damage if the enemy's weakness is Ice. However, some enemies nullify or absorb certain elements, and the same thing will happen to your attack.

When this link is used on armor, a different effect happens. Your character becomes resistant to that element. At Level 1, the materia only halves the damage taken; at Level 2, the materia simply nullifies it (does zero damage); and at Level 3 or Mastered, these elemental attacks will actually heal the character.

You can check any of these by accessing the character's Status menu and pressing the [OK] button once.

Elemental is incompatible with Master Spell materia.

Final Attack

Where to find this
This materia is one of the most sought-after of all time, as it is a big help when fighting Emerald and Ruby weapons. Here's how to get it. In Disc 3, go to the Battle Square at the Gold Saucer. You would need to win the W-Summon materia and the Omnislash limit (and learn it!) in order to be offered the Special Battle by the lady at the counter. In the Special Battle, you'll face hard enemies such as the last enemies of the Regular Battle and others (like Unknown 3), and finally, the Proud Clod. You will get Final Attack as the reward after beating the entire Special Battle. Many people have had trouble beating this all the way, but a level 83 Cloud with Ultima Weapon, Ribbon, maxed out materia and hopped up on Sources, as well as 9999 HP and 99 MP, will be able to get through this fairly easily (those were my stats).
Additional Notes
Final Attack, in case you did not know, allows you to cast one final attack (from a paired materia) before dying. The most useful combo with this is something that can raise that character (or entire party) back to life. One of the best ways is to pair it with a Phoenix summon materia, which will keep your whole party alive and do damage. Revive also works well.
Final Attack will work only when an enemy kills the one with it equipped. You can't attack him with your own characters; that doesn't work.

HP Absorb

Where to find this
The first of two of these is found in Wutai. You must first finish Yuffie's sub-quest here, then return to the cat house. You can now go upstairs. There is a chest up there which will give you the HP Absorb.
The second is hidden in plain sight when you enter the Northern Cave. You'll see it flashing in the wall, but you can't reach it. It's simpler than you might think. Climb up to the platform on the left, where there's a chest. After getting whatever it is that's in that chest, go toward the back and right of the ledge. You should walk through a hole in the wall there and then be able to pick up the HP Absorb materia.

Additional Notes
Every time you do damage with whatever is linked to this, you will regain 1/10 of the damage dealt. For example, say you linked HP Absorb with Fire. Casting Fire may do about 400 damage, and you will regain 40 HP. Obviously you will want to link this with whatever spells do the most damage, such as Demi or Knights of the Round.
Be forewarned, though, that if the linked spell actually HEALS the target, then you would LOSE HP instead. For instance, Master Tonberry absorbs Demi. Casting Demi on him will heal him 9999 points, and deal 999 damage to you. Not good at all. Linking HP Absorb with a Restore or Full Cure materia is just about the dumbest thing you can possibly do. (If you heal yourself 100 HP, you lose 10, for a net of 90 HP.)

HP Absorb does not work with regular command materia, but will work with a Master Command materia. However, it affects only Morph, D.blow, and Coin. An odd trick is that if you paired HP Absorb to Master Command, and then put another command materia in a slot elsewhere that is not in the Master, the HP Absorb may work with that command as if it was on the Master! Weird.

Get it at the Materia Shop in Wutai. Yuffie will steal it (it is part of the subquest) but she will return it later.
Additional Notes

This materia is one of the rarer prizes to be gotten when you win an S-Class Chocobo race at the Golden Saucer.
Additional Notes
Sneak Attack does not always work every single battle (although its occurence rate may go up with materia level). What it's supposed to do is cast the best spell (Magic materia only, apparently) on a paired materia at the beginning of a battle. You'll know it'll happen when a message appears at the top of the screen telling you the character "was caught by surprise." The character will cast helpful spells on the party and attack spells on the enemy. Usually this happens on a random target, but with Mega All equipped, the spell will hit either all enemies or all allies.

Found in the fire caves in the Da-Chao statues of Wutai. You'll need the Leviathan Scales (pick them up in Junon right before the submarine game) in order to get rid of the flames.

Quadra Magic

Where to find this
Found in the Materia Cave on an island north of Mideel. It is accessible with a Blue, Black or Gold Chocobo. (You'll need to maneuver over and around many islands to get there with a Blue, but it's possible.)
Additional Notes
Quadra Magic is a powerful materia which allows you to cast any paired magic spell or summon monster four times in a row. (Yes, it does work with Summons, except for Knights of the Round.) The coolest thing? The MP cost is still as if you have only cast it once! The Quadra Magic can only be used the same number of times as its level, however (similar to a summon materia).
