The Adjutant General U.S.A. reports the following military history of the soldier:
W.H. Bolar mustered in as 1st Lieut. C. 19th Ky. Vol. Jan 2, 1862 and as Capt. William H. Bolar to date Apr. 2, 1863 Rolls to June 30, 1863 "present" roll for July & Aug 63 Absent on 20 days furlough commencing Aug 2, 1863. Roll for Sept & Oct 63 present. Subsequent rolls to June 30, 1864 Present. Honorably discharged July 27, 1864 upon tender of resignation.

Has suffered from first mentioned disease since Feb. last. Will not be able to resume his duties in a less period than five months.
