CHILD IDENTIFICATION KIT:
The information contained in this section will provide you with a valuable and compresensive identification and tracting system.
HOW TO USE THIS KIT:
Should your child ever become missing or lost, immediately take this kit to your local law enforcement agency and request that the information be entered immediately into the FBI's National Crime Information Center.
REMEMBER:
Keep the information in this kit current and update it once a year. Keep the CHILD INFORMATION KIT in a safe and easily accessible place. When you and your family go away, it is advisable to take this kit with you.
WHAT'S INCLUDED IN YOUR KIT:
1. Vital information.
2. Your child's fingerprints.
3. Annual photograph of your child.
4. Lock of your child's hair.
5. Your child's growth record.
6. A copy of your child's dental records.
7. A copy of your child's medical records.
8. Emergency information.
BASIC INSTRUCTIONS:
1. Start out with a white 8.5 x 11 inch piece of paper or card stock and fold it in half forming a small booklet for EACH of your children.
2. Label the top of your booklet CHILD IDENTIFICATION KIT.
3. On the front cover write or type in the following Vital Information:
VITAL INFORMATION:
1. Last name, middle, first
2. Address
3. Date of birth (include month, day and year)
4. Indicate the sex of your child (male or female)
5. Include the race of your child
ANNUAL PHOTOGRAPH:
1. Either take an annual photograph of your child or use one of the annual school portraits that most schools offer each year. Tape the picture in the upper left hand side of page 2 of your CHILD IDENTIFICATION KIT.
2. The next year, move last year's photograph to the right side of page 2 and tape the "new" current photograph to the left side.
3. Label and keep older photographs (past years photographs) loosely inside your CHILD IDENTIFICATION KIT.
4. By keeping and maintaining a sequential group of photographs in each child's CHILD IDENTIFICATION KIT, authorities and police sketch artists can anticipate what your child may look like many years or decades into the future should there ever be a problem locating your child for a prolonged period of time. Of course, no parent wants to ever think of this happening to them, but our goal here is to be PROACTIVE and prepared for any unseen circumstances.
5. As a helpful reminder to update your child's annual picture, pick a date you can remember, such as their birthday or January 1st of each "new" year and stick to it.
FINGERPRINTS:
You are about to draw a series of small boxes on the lower portion of page 2 of your CHILD IDENTIFICATION KIT to record your child's fingerprints.
1. On the lower portion of page 2 of your CHILD IDENTIFICATION KIT draw a rectangular box occupying about 1/3 of the lower portion of the page.
2. Now draw a horizontal line through the middle of your rectangular box dividing it in half horizontally.
3. At this point draw four vertical lines equally spaced within the rectangle creating five small square boxes on the top portion of the rectangle, and five square boxes on the bottom portion of the rectangle for a total of ten boxes.
4. At the lower portion of each of the top squares (moving from left to right) write the following:
- Right thumb
- Right index
- Right middle
- Right ring
- Right little
5. At the lower portion of each of the bottom square (moving from left to right) write the following:
- Left little
- Left ring
- Left middle
- Left index
- Left thumb
6. Obtain a black ink pad.
7. Beginnig with your child's right thumb, roll it in the inkpad ink. Immediately roll it in the appropriate square on your CHILD IDENTIFICATION KIT.
8. Follow the same procedure mentioned above for each finger. Roll each finger from edge to edge making a square-shaped print.
9. After completing the right hand, move to the left hand and do the same.
LOCK OF YOUR CHILD'S HAIR:
1. Cut off a small lock of your child's hair and place it in a small baggie. It is now possible for authorities to make identification through your child's DNA which can be tested on the locks of his or her hair.
2. Place the filled baggie with the lock of your child's hair into the center fold of the CHILD IDENTIFICATION KIT.
GROWTH RECORD:
1. In the upper left hand section of page 3 of your CHILD IDENTIFICATION KIT draw a square box.
2. Draw five equally spaced horizontal lines within the box.
3. Now draw two equally spaced vertical lines within the box.
4. At this point you should have created 18 small rectangular boxes within the larger square. You should see three columns.
5. In the top-left small rectangle write or type the word DATE.
6. In the top-middle small rectangle write or type the word HEIGHT.
7. In the top-right small rectangule write or type the word WEIGHT.
8. Each year, at the same time you update your child's picture, record the current information as of the posted date. Keep this information current.
MEDICAL RECORD:
Include the following medical information on page three of your CHILD IDENTIFICATION KIT:
- Hair color
- Eye color
- Blood type
- Whether their ears are pierced
- Medical conditions
- Allergies
- Scars location identification
- Birthmark location identification
- Mole location identification
- Any other unique identification features on your child
DENTAL RECORDS:
1. Obtain a complete set of dental x-rays from your child's dentist and place them in your CHILD IDENTIFICATION KIT.
2. Authorities can often identify a person from their dental records.
EMERGENCY INFORMATION:
On the back portion of your CHILD INFORMATION KIT record the following telephone numbers:
- Police department
- Fire department
- Nearest three hospitals
- Daycare center
- Poison control center
- Other important telephone numbers
YOU'VE DONE IT:
It took a little doing but you've created your own CHILD IDENTIFICATION KIT. We hope you'll never have to use it, but just in case you do, it's wise to have this important information up-to-date and at your fingertips. Many times, time is your most important tool, and the quicker you can get this vital information to authorities, the better your chances.
